Question

68-year-old patient complaining of left wrist pain on thumb extension (no obvious trauma).

Ultrasound: Fluid layer in the sheath of the extensor pollicis brevis and abductor pollicis longus tendons. No intra-articular effusion or edema in the cutaneous and subcutaneous soft tissues. No hypervascularization on Doppler.

What is the next step?

ANSWER

This is De Quervain’s tenosynovitis, which warrants an ultrasound-guided injection.
